INSURANCE REMINDERS
To help avoid delays and ensure accurate billing, please keep the following in mind:
• Bring Your Insurance Card: Present your most current insurance card at every visit.
• Update Us on Changes: If your insurance coverage changes, bring your new card so we can update our records.
• Check Immunization Coverage: Before vaccination appointments, confirm with your insurance provider that your child’s immunizations are covered.
• Notify Your Employer/Insurer: Let them know about any new additions to your family to ensure timely coverage.

PAYMENT & BILLING POLICY
Copay Collection
Copays are collected at check-in to help you leave promptly after your appointment—especially helpful during sick visits.
Payment Methods
Payment is due at the time of service. We accept:
• Cash
• Checks
• All major credit cards
• Online payments via HealowPay.com
Your copay amount is determined by your insurance provider, so please bring your card to every visit.
ACCOUNT BALANCES & STATEMENTS
• You may be reminded of any outstanding balance during your visit.
• Balances are due within 30 days of your statement date.
• We offer CareCredit for balances over $200.
• In-house payment plans are available for select balances.
• Deductibles, coinsurance, and cost shares are due after your statement is issued. We reserve the right to collect deductibles at the time of service.
